Travel Social Worker in Mission Viejo, CA

A Travel Social Worker Master of Social Work will provide discharge planning and care coordination in an acute care hospital in Mission Viejo, California on a 13-week contract. The unit serves adults and geriatrics, with an average caseload of 20:23 and Epic charting.

• Complete initial discharge assessments
• Coordinate discharge plans and post-acute needs
• Collaborate with medical teams and case managers
• Arrange referrals and community resources
• Educate patients and caregivers
• Document care plans and progress

• Master of Social Work
• BLS
• 2 years of acute care inpatient hospital experience
• Recent hospital experience
• Epic charting experience

• LCSW
• Trauma Level I or II experience
• Experience with stroke, ICU, NICU or pediatric patients
• Experience with dementia, COPD, CHF or complex diagnoses
