Travel Social Worker in Edmonds, WA

A Travel Social Worker Licensed Master Social Worker will support case management in an acute care hospital on a 13-week contract in Edmonds, Washington. The unit serves infants, adults and geriatrics, averages 18-20 patients, and uses Epic charting. First-time travelers are welcome.

• Complete social work assessments and identify discharge needs
• Develop care plans and coordinate discharge planning
• Arrange home care, equipment and community resources
• Document assessments, interventions and discharge summaries
• Collaborate with nurses, providers and support services
• Educate patients and caregivers on follow-up and advance directives
• Participate in care conferences and referral coordination

• Licensed Master Social Worker credential
• Master of Social Work degree
• BLS certification
• 2 years of acute care med surg experience
• Hospital experience
• Epic charting experience

• ICU experience
• Stroke experience
• Dementia and psychiatric disorder experience
• Concurrent review and prior authorization familiarity
