Travel Social Worker in Seattle, WA

A Travel Transplant Social Worker Licensed Independent Clinical Social Worker will support pre- and post-liver transplant patients in an outpatient hospital setting on a 13-week contract in Seattle, Washington. The role serves adults, uses Epic charting, and may also complete inpatient evaluations as needed.

• Complete psychosocial evaluations for pre- and post-liver transplant patients
• Verify caregiver support by phone and coordinate follow-up needs
• Document assessments, care plans, goals, and patient education
• Provide individualized education to patients and caregivers
• Participate in care conferences and team planning
• Assist with inpatient evaluations when department needs require

• Master of Social Work degree
• 1 year of short term acute care inpatient medical social worker experience
• Washington Licensed Independent Clinical Social Worker license
• Biopsychosocial assessment, documentation, and patient education skills

• Epic charting system experience
• Transplant experience
• Emergency department, ICU, or medical surgical background
