Travel Social Worker in Anchorage, AK

A Travel Social Worker, Licensed Masters Social Worker, will provide consult-based psychosocial support and discharge planning in an acute hospital in Anchorage, Alaska on a 26-week contract. The role supports emergency, children’s and adult teams in a Magnet, Level II trauma hospital using Epic.

• Complete psychosocial assessments for patients and families
• Develop person-centered discharge plans and contingency plans
• Provide brief clinical interventions and crisis support
• Screen for suicidality and complete safety planning
• Coordinate resource navigation and care transitions
• Communicate with RN case managers and outside entities
• Document reports of harm and care coordination details

• Alaska Licensed Masters Social Worker or temporary Alaska license
• Master's degree in social work from an accredited program
• 2 years of acute care social work experience
• Psychosocial assessment and discharge planning skills

• LCSW
• Epic charting system experience
• Emergency Department experience
• Pediatrics experience
