Travel Social Worker in Torrance, CA

A Travel Social Worker Master of Social Work will provide care coordination and discharge planning in an acute care hospital for a 13-week contract in Torrance, California. The role supports a case load up to 25 patients with a 1:35 ratio across neonates, infants, pediatrics, adolescents, adults, and geriatrics, with Epic charting.

• Provide care coordination and discharge planning
• Complete biopsychosocial assessments and intake screenings
• Organize family meetings and care conference participation
• Develop treatment plans with the interdisciplinary team
• Arrange referrals to home care, rehab, long-term care, and community resources
• Document patient and caregiver education and care coordination in the record

• Master of Social Work degree
• 2 years of acute hospital case management experience
• Epic charting system experience
• Prior travel experience

• California social work license or pending licensure
• Experience with behavioral health, homelessness, and substance use cases
• Knowledge of Medicaid, Medicare, prior authorization, and community resources
• Experience with psychiatric, ICU, pediatrics, or emergency care settings
