Travel Physical Therapist in Grand Junction, CO

A Travel Physical Therapist will provide in-home therapy for patients in Grand Junction, Colorado on a 13-week contract. The role focuses on one-on-one visits, Homecare Homebase documentation, and helping patients improve mobility, safety, and independence in a home health setting.

• Perform evaluations, reassessments, recertifications, and discharge visits
• Develop individualized plans of care based on patient needs and orders
• Deliver gait training, balance training, therapeutic exercise, and mobility training
• Teach patients and caregivers home exercise and safety strategies
• Document visits accurately in Homecare Homebase
• Coordinate patient progress and changes in condition with the care team
• Support PTAs in clinical oversight when permitted by law and policy

• Current, unrestricted Colorado Physical Therapist license
• Graduation from an accredited Physical Therapy program
• Valid driver’s license and reliable transportation
• Ability to practice independently in a home-based setting
• Homecare Homebase proficiency or willingness to learn

• Home health experience
• Familiarity with Medicare documentation and home health regulations
• Experience with geriatric and medically complex patients
