A Travel Occupational Therapist will provide rehabilitative evaluation and treatment for residents in a nursing home during a 13-week contract in Oklahoma City, Oklahoma. Services focus on cognitive and ADL-related functioning, with electronic documentation and collaborative care planning.

• Screen and evaluate residents with cognitive and ADL-related functional needs
• Develop and adjust individualized plans of care
• Provide skilled treatment using therapeutic activities, exercise, modalities, and assistive devices
• Document treatment progress, minutes, and discharge summaries in the electronic record
• Communicate progress, barriers, and plans with the care team and families
• Support care conferences, utilization review, and supervision of assistants or students

• Oklahoma Occupational Therapist license
