Travel Occupational Therapist in Medford, OR

A Travel Occupational Therapist will provide home health care in Medford, Oregon on a 13-week contract. The role uses Oasis charting and may see up to 6 patients per day with grouped visits that reduce drive time.

• Evaluate functional status and home safety needs
• Develop individualized occupational therapy plans of care
• Provide therapy to improve daily living and self-care skills
• Teach patients and caregivers adaptive techniques and equipment use
• Document visits and progress in charting systems
• Coordinate care with the interdisciplinary team
• Monitor response to treatment and adjust interventions

• Oregon Occupational Therapist license
• BLS certification
• 1 year of skilled home health occupational therapy experience
• Experience with electronic charting systems, including Oasis
• Graduation from an accredited occupational therapy program
