Travel Case Management RN in West Hollywood, CA

A Travel Case Management Registered Nurse will coordinate care in an acute hospital case management unit on a 13-week contract in West Hollywood, California. The unit serves adolescents, adults, and geriatrics, with an average daily census of 255 and Epic charting.

• Coordinate care plans across admission, stay, and discharge needs
• Review utilization and medical necessity for acute care stays
• Collaborate with social services and transportation support
• Develop discharge plans and document progress in Epic
• Communicate with providers about barriers, resources, and follow-up care
• Support care coordination for adolescents, adults, and geriatrics

• Active California Registered Nurse license
• BLS certification
• 3 years of acute care case manager experience
• Utilization management experience
• Acute hospital experience
• Care coordination, discharge planning, and plan of care skills

• Epic charting system experience
• InterQual and Milliman criteria experience
• CMS, DRG, and core measure knowledge
• Teaching or community hospital experience
