A Travel Case Management Registered Nurse will coordinate patient care across the hospitalization in an acute care hospital on a 13-week contract in Glendale, California. The role acts as a liaison among patients, families, nurses, physicians, and the multidisciplinary team to support smooth transitions from admission through discharge.
