Travel Case Management RN in Apple Valley, CA

A Travel Case Management Registered Nurse will coordinate care and discharge planning in an acute care hospital in Apple Valley, California on a 13-week contract. The unit serves neonates, adolescents, adults, and geriatrics and uses Epic for computerized documentation.

• Assess patient needs and discharge barriers
• Coordinate care plans with the interdisciplinary team
• Review utilization and admission criteria
• Arrange DME, SNF placement, and ambulance transport
• Facilitate family meetings and treatment planning
• Support patients with complex psychosocial needs
• Collaborate with social services and physical therapy

• Active California Registered Nurse license by start date
• 2 years of acute care case management and discharge planning experience
• Prior travel experience
• Epic charting experience
• Care coordination, utilization review, and discharge planning skills
• DME, SNF, and ambulance transport coordination skills
