Travel Case Management RN in Apple Valley, CA

A Travel Case Management Registered Nurse will coordinate assessments, discharge planning, and care transitions in an acute care hospital on a 26-week contract in Apple Valley, California. The unit serves neonates through geriatrics, averages 25 patients daily, and uses Epic.

• Assess patient needs and support discharge planning
• Coordinate care transitions and utilization review
• Participate in family meetings and treatment planning
• Arrange DME, SNF placement, and ambulance transport
• Collaborate with social services, therapy, and community resources
• Document case management activity in the charting system

• California Registered Nurse license, active by start date
• 2 years of acute care case management or discharge planning experience
• Prior travel experience
• Care coordination, discharge planning, and utilization review skills
