A Travel Case Management Registered Nurse will coordinate utilization review, care planning, discharge planning, and resource management in a long-term acute care hospital on a 12-week contract in Paramount, California. The role supports interdisciplinary collaboration, patient advocacy, and continuity of care across acute care settings.

• Assess patient needs and support comprehensive care planning
• Coordinate utilization review and resource management
• Collaborate with the interdisciplinary team on care decisions
• Implement discharge plans and monitor progress toward goals
• Advocate for patient preferences and continuity of care
• Support quality of care and efficient service delivery

• California Registered Nurse license
• Basic Life Support certification through the American Heart Association
• 2 years recent RN Case Management experience in LTAC or STACH settings
• MCG experience and discharge planning across acute care settings
• Strong interdisciplinary team collaboration
