Travel Case Management RN in Mission Viejo, CA

A Travel Case Management Registered Nurse will provide care coordination and discharge planning in an acute care hospital on a 26-week contract in Mission Viejo, California. The unit serves adults and geriatrics, uses Epic for computerized documentation, and manages an average daily census of up to 25.

• Assess patient needs for care coordination and discharge planning
• Develop and participate in care plans with the team
• Arrange DME, SNF placement, and ambulance transport
• Lead family meetings and support communication with patients and families
• Address appeals, denials, and utilization concerns
• Document case management activities in Epic

• California Registered Nurse license, active or pending with licensure upon start date
• 2 years of recent acute medical care manager experience performing care coordination
• Epic charting system experience
• Prior travel experience

• Emergency Department, ICU, or Surgical Services experience
• Trauma Level I or Trauma Level II experience
