Travel Case Management RN in Mission Viejo, CA

A Travel Case Management Registered Nurse will coordinate care, discharge planning, and treatment planning for adults and geriatrics in an acute care hospital in Mission Viejo, California on a 13-week contract. The unit uses Epic, manages up to 25 patients daily, and serves complex cases that need care coordination and family involvement.

• Coordinate care and discharge planning
• Assess patient needs and barriers to safe discharge
• Arrange DME, SNF placement, and ambulance transport
• Participate in family meetings and treatment plan development
• Collaborate with payers and interdisciplinary teams
• Support cases involving mental health, substance use, homelessness, and protective services

• California Registered Nurse license
• 2 years of acute medical care management experience with care coordination
• Epic charting system experience
• Prior travel experience
• Licensed at start date if pending

• Emergency Department experience
• ICU experience
• Surgical Services experience
• Trauma Level I or Level II experience
