A Travel Case Management Registered Nurse will coordinate care and discharge planning in an acute care hospital for a 13-week contract in Glendale, California. The role supports a defined patient population from pre-admission through post-discharge and serves as a liaison for patients, families and the care team.

• Coordinate care across admission, hospitalization and discharge
• Collaborate with patients, families, nurses, physicians and the interdisciplinary team
• Act as a liaison to support communication and patient needs
• Consult on case management concerns and care planning
• Monitor progress toward safe, efficient transitions of care
• Support case management reporting and program revision
