Travel Case Management RN in Sitka, AK

A Travel Case Management Registered Nurse will provide patient-centered care coordination in an outpatient clinic setting on a 13-week contract in Sitka, Alaska. The role supports patients and families across the continuum of care, with documentation in the electronic health record and collaboration with providers and interdisciplinary teams.

• Assess patients and develop individualized care plans
• Coordinate referrals, follow-up care, and continuity of services
• Educate patients and families on care plans, medications, and discharge needs
• Document assessments, interventions, and outcomes in the electronic health record
• Collaborate with providers and interdisciplinary team members
• Monitor patient responses and escalate changes in condition
• Support quality, safety, and infection prevention practices

• Active Alaska Registered Nurse license
• Basic Life Support certification
• Graduate of an accredited school of nursing, high school diploma or equivalent
• 1 year of nursing experience

• Case Management certification
• Bachelor's degree
