Travel Case Management RN in San Leandro, CA

A Travel Case Management Registered Nurse will coordinate care, utilization review, and discharge planning in a long-term acute care hospital in San Leandro, California on a 13-week contract. The role supports interdisciplinary collaboration, patient advocacy, and continuity of care for complex patients.

• Assess patient needs and coordinate care plans
• Monitor utilization of resources and service delivery
• Develop and update discharge plans
• Collaborate with the interdisciplinary team to support transitions of care
• Advocate for patient preferences and holistic needs
• Evaluate outcomes and adjust plans as needed

• California Registered Nurse license
• BLS certification through the American Heart Association
• 2 years of recent RN Case Management experience in LTAC or STACH settings
• MCG experience
• Discharge planning experience across acute care settings
• Strong interdisciplinary team collaboration skills
