Travel Case Management RN in California

A Travel Case Management Registered Nurse will coordinate care and discharge planning for adults and geriatrics in an acute care hospital in Burbank, California for a 26-week contract. The unit uses Epic, manages a case load up to 25, and welcomes first-time travelers.

• Assess patient needs and discharge barriers
• Coordinate care transitions with the interprofessional team
• Review medical necessity and admission criteria
• Arrange DME and post-acute services
• Document care coordination in Epic
• Support managed care communication and utilization review

• California Registered Nurse license by start date, pending accepted
• 2-year acute medical case management experience
• Care coordination and discharge planning experience
• Epic charting system experience
• Prior travel experience
• Knowledge of admission criteria, medical necessity review, DME planning, CMS, DRG, and HIPAA guidelines
