Travel Case Management RN in Mission Hills, CA

A Travel Case Management Registered Nurse will coordinate care and discharge planning in an acute care hospital in Mission Hills, California for a 26-week contract. The role supports adolescents, adults and geriatrics, uses Epic and manages a 1:20 assignment. First-time travelers are welcome.

• Assess patient needs and care coordination priorities
• Develop and update discharge plans with the care team
• Coordinate family meetings and treatment plan discussions
• Arrange DME, SNF placement and ambulance transport
• Collaborate with social services and community resources
• Document case management activities in computerized systems
• Apply InterQual criteria and support plan-of-care decisions

• California Registered Nurse license, active before start
• 2-year acute care hospital case management experience in care coordination and discharge planning
• Epic charting system experience
• Prior travel experience

• DRG diagnosis-related groups
