Travel Case Management RN in Mission Hills, CA

A Travel Case Management Registered Nurse will coordinate care and discharge planning in an acute care hospital on a 13-week contract in Mission Hills, California. The caseload is about 1:20, the unit serves adolescents, adults, and geriatrics, and Epic is used for documentation. First-time travelers are welcome.

• Assess patient needs and discharge barriers
• Coordinate care across the treatment team
• Develop discharge plans and care transitions
• Arrange DME, SNF placement, and ambulance transport
• Lead family meetings and support care planning
• Document case management activity in Epic

• California Registered Nurse license, active by start date
• 2 years of acute care hospital case management with care coordination and discharge planning
• Epic charting system experience
• Prior travel experience

• DRG knowledge
• InterQual criteria experience
