Travel Case Management RN in Pasadena, TX

A Travel Case Management Registered Nurse will support case management and social work services in a hospital setting on a 13 week contract in Pasadena, Texas. The department covers the entire hospital, uses Meditech and Meditech Expanse, and manages about 20 to 25 patients per RN.

• Assess discharge needs and plan safe transitions of care
• Review InterQual criteria to support patient status determinations
• Coordinate with social work and the care team on placement needs
• Communicate care plans and discharge steps to patients and families
• Document case management activities in the electronic record
• Support hospital-wide utilization review and discharge planning

• Texas Registered Nurse license
• 2 years of hospital case management experience
• Discharge planning and utilization review skills
• Experience with InterQual criteria
• Proficiency with Meditech or Meditech Expanse
