Travel Case Management RN in El Paso, TX

A Travel Case Management Registered Nurse will coordinate inpatient and observation care transitions in an acute care hospital on a 13-week contract in El Paso, Texas. The unit uses MEDITECH, MIDAS, CAREPORT, and InterQual, with 25:1 case management coverage and 40:1 social work coverage.

• Assess psychosocial, medical, and discharge needs
• Coordinate care plans with the multidisciplinary team
• Guide discharge planning for inpatient and observation patients
• Review progression of care using InterQual
• Communicate care needs and referrals with patients and families
• Support collaboration with case managers and social workers

• Texas Registered Nurse license
• 2 years of acute case management experience
• Discharge planning and utilization review skills
