A Travel RN Case Management Registered Nurse will coordinate care and discharge planning in a long-term acute care hospital on a 13-week contract in Paramount, California. The role supports utilization review, interdisciplinary collaboration, and continuity of care.

• Assess patient needs and develop coordinated care plans
• Support utilization review and resource management
• Collaborate with the interdisciplinary team on care transitions
• Update discharge plans and continuity-of-care goals
• Advocate for patient preferences and holistic needs
• Evaluate outcomes and adjust plans as needed

• California Registered Nurse license
• Basic Life Support certification
• 2+ years of recent RN case management experience
• LTAC or STACH experience
• MCG and discharge planning experience across acute care settings
• Strong interdisciplinary collaboration skills
