Travel Case Management RN in Kansas City, MO

A Travel Case Management Registered Nurse will coordinate discharge planning and continuum of care in an acute care hospital on a 12-week contract in Kansas City, Missouri. The unit manages a variable 1:25 caseload and uses Meditech, Midas, and WellSky. RN, social work, and CMA support the work.

• Coordinate discharge planning and transition of care for hospitalized patients
• Assess needs, barriers, and follow-up services for safe discharge
• Collaborate with physicians, social work, nursing, and ancillary teams
• Document case management activity and care plans in the chart
• Connect patients with community resources and post-acute services
• Monitor utilization, length of stay, and discharge readiness

• Missouri Registered Nurse license
• Recent acute care discharge planning experience
• Graduation from an accredited nursing program
• Care coordination and discharge planning skills
• Interdisciplinary communication and documentation skills
