Travel Case Management RN in Fort Worth, TX

A Travel Case Management Registered Nurse will coordinate care and discharge planning in an acute care hospital in Fort Worth, Texas on a 13-week contract. The neuro step-down unit has about 40 beds, typical ratios of 1:20-30, and uses Meditech and Careport.

• Coordinate patient care transitions and discharge plans
• Assess utilization, continued stay, and care needs
• Collaborate with nurses, social workers, and providers
• Document case management actions in Meditech and Careport
• Support referrals and community resource placement
• Manage caseloads for complex neuro step-down patients

• Active Texas Registered Nurse license
• 1 year of acute care case management experience
• Strong discharge planning and care coordination skills

• Meditech experience
• Careport experience
