Travel Case Management RN in Everett, WA

A Travel Case Management Registered Nurse will support discharge planning and care coordination in an acute care hospital in Everett, Washington for a 13-week contract. The role uses Epic in a Level II trauma center and serves patients across all ages. First-time travelers are welcome.

• Coordinate discharge planning and care coordination for acute care patients
• Assess patient needs and identify next level of care
• Arrange DME and support plan of care updates
• Collaborate with social services and the care team
• Document case management activity in Epic
• Adapt care coordination for patients across all age groups

• Washington Registered Nurse license
• BLS certification
• 1 year of acute hospital experience
• Discharge planning and care coordination experience
• Med Surg experience
• Epic charting experience

• Emergency Department case management experience
• ICU case management experience
• Long length of stay case management experience
