Travel Case Management RN in Charlottesville, VA

A Travel Registered Nurse Case Manager will coordinate inpatient case management in an acute care hospital for a 13-week contract in Charlottesville, Virginia. The unit serves adults and geriatrics and uses Epic.

• Assess patient needs and care coordination goals
• Coordinate discharge planning with the care team
• Review utilization and transition needs for acute care patients
• Arrange post-acute services, equipment, and resources
• Document case management activities in Epic
• Provide patient and family education on care plans and follow-up

• 2 years of acute care hospital inpatient case management experience
• Epic charting experience or travel experience
• Virginia Registered Nurse license, pending accepted
• Care coordination and discharge planning skills

• BLS
• MS degree
• Disease management, needs assessment, DME ordering, and plan of care experience
• Teaching hospital experience and trauma level I or II experience
