Travel Case Management RN in Anchorage, AK

A Travel Case Management Registered Nurse will support discharge planning and care coordination in an acute hospital on a 26-week contract in Anchorage, Alaska. The unit serves neonates through geriatrics and uses Epic charting. First-time travelers are welcome.

• Coordinate discharge planning and care transitions
• Assess patient needs and develop plans of care
• Review continued stay needs and medical necessity
• Arrange DME, referrals, and follow-up services
• Collaborate with physicians, social services, and therapy teams
• Document care management activities in Epic

• Alaska Registered Nurse license
• 2 years of acute care hospital case management experience
• Epic charting system experience
• Care coordination, discharge planning, continued stay review, and medical necessity review skills

• BLS
• ACM certification
• Microsoft Office and Teams experience
• InterQual and Milliman guideline utilization
