Travel Case Management RN in Anchorage, AK

A Travel Case Management Registered Nurse will support discharge planning and care coordination in an acute hospital in Anchorage, Alaska on a 26-week contract. The unit uses Epic and serves infants through geriatrics in a Magnet, Level II trauma setting. First-time travelers are welcome.

• Assess patient needs and discharge barriers
• Coordinate discharge planning and continued stay reviews
• Review medical necessity and plan of care
• Arrange DME, referrals, and community resources
• Document case management updates in Epic
• Collaborate with the interdisciplinary care team

• Alaska Registered Nurse license
• 2 years of acute hospital case management experience
• Epic charting system experience
• Care coordination, discharge planning, continued stay review, and medical necessity review skills

• BLS
• ACM-RN certification
• InterQual and Milliman criteria experience
• Community hospital, LTAC, or Level II trauma experience
