Travel Case Management RN in Barre, VT

A Travel Case Management Registered Nurse will provide individualized, goal-oriented nursing care in a home health and hospice setting in Barre, Vermont for a 20-week contract.

• Assess patient needs and establish individualized care plans
• Provide nursing care in accordance with physician orders and the nursing process
• Coordinate care with physicians and the interdisciplinary team
• Serve as case manager when assigned as lead discipline
• Monitor patient status and update plans based on changes
• Document care and communicate patient progress

• Active Vermont Registered Nurse license
• BLS/BCLS certification
• 1 year of nursing experience
