Travel Social Worker in Santa Monica, CA

A Travel Social Worker Master of Social Work will support care coordination, discharge planning, and crisis intervention in a hospital social work unit for a 13-week contract in Santa Monica, California. The role serves neonates through geriatric patients, manages up to 22-25 daily visits, and uses Epic.

• Assess psychosocial needs and barriers to care
• Coordinate placement, community resources, and follow-up services
• Support crisis intervention and safety planning
• Participate in care conferences and family meetings
• Develop, update, and document care plans and treatment goals
• Communicate with internal and external team members

• Master of Social Work degree
• 2 years of acute medical hospital inpatient social work experience
• Epic charting system experience
• Prior travel experience
• Care coordination, biopsychosocial assessment, and discharge planning skills

• LCSW
• Experience with involuntary holds and emergency department cases
• Knowledge of substance use treatment options and InterQual criteria
• Familiarity with NICU, L&D, mother-baby, and Medicare waiver workflows
