Travel Social Worker in Oregon City, OR

A Travel Social Worker Licensed Clinical Social Worker will provide behavioral health assessments and care coordination in an acute care hospital on a 26-week contract in Oregon City, Oregon. The unit uses Epic, serves patients of all ages in the emergency department, and welcomes first-time travelers.

• Complete biopsychosocial assessments in the emergency department
• Coordinate discharge planning and community resource referrals
• Support behavioral health evaluations for patients of all ages
• Collaborate with nurses, providers and case managers
• Document care coordination, treatment planning and follow-up needs
• Assist with protective services, guardianship and family support needs

• Oregon Licensed Clinical Social Worker license
• BLS certification
• 1 year of social work experience
• Charting system experience

• Acute behavioral health assessment experience
• Care management experience
