Travel Social Worker in Mission Viejo, CA

A Travel Medical Social Worker Master of Social Work will manage discharge planning and case management in an acute care hospital for a 13-week contract in Mission Viejo, California. The unit serves adults and geriatrics, with a caseload of 20-23 and Epic charting.

• Complete initial assessments within 24 hours
• Develop and update discharge plans
• Coordinate post-acute care needs
• Collaborate with the medical team and case managers
• Educate patients and caregivers on discharge needs
• Document assessments, plans, and care coordination in Epic

• Master of Social Work
• BLS certification
• 2 years of recent acute care inpatient hospital experience
• Epic charting system experience

• LCSW
• Trauma Level I or Trauma Level II experience
• Stroke experience
