Travel Social Worker in Anchorage, AK

A Travel Social Worker Master’s Social Worker will provide case management support in a hospital case management unit on a 13-week contract in Anchorage, Alaska. The role uses EPIC and MIDAS, with a 1:8 ratio and participation in discharge planning and multidisciplinary rounds.

• Assess social needs and discharge barriers
• Develop care plans from psychosocial assessments
• Participate in multidisciplinary discharge planning rounds
• Consult with healthcare professionals on patient needs
• Document interventions and care plans in the chart

• Active Alaska social work license
• LMSW
• Master of Social Work or equivalent degree
• EPIC experience
• MIDAS experience
