Travel Social Worker in Oregon City, OR

A Travel Social Worker Licensed Clinical Social Worker will provide behavioral health assessment and care coordination in an acute care hospital on a 26-week contract in Oregon City, Oregon. The role supports emergency department behavioral health work, discharge planning, Epic charting, and a 1:16 caseload.

• Complete psychosocial and behavioral health assessments in the emergency department
• Coordinate discharge planning and care transitions with the care team
• Develop treatment plans and participate in care conferences
• Provide individual, family, and crisis counseling
• Connect patients with community resources and protective services
• Document findings, referrals, and service coordination accurately

• Oregon Licensed Clinical Social Worker license
• BLS certification
• 1 year of social work experience
• Experience with electronic charting systems

• Emergency department behavioral health assessment experience
• Care management and discharge planning experience
• Acute behavioral health assessment experience across all ages
