A Travel Social Worker Licensed Clinical Social Worker will support patients and families in an acute hospital in Lawrenceville, Georgia on a 12-week contract. The role focuses on psychosocial support, discharge planning, and care coordination.

• Assess psychosocial needs and barriers to discharge
• Support patients and families with counseling and resource planning
• Coordinate care with nurses, physicians, and case management
• Develop discharge plans and community referrals
• Document interventions, progress, and follow-up needs
• Respond to crisis situations and care transitions

• 2 years of social work experience in a specialty setting
• Current Georgia Licensed Clinical Social Worker license
• AHA BLS certification
• Acute hospital experience
• Clinical assessment, discharge planning, and care coordination skills
