Travel Social Worker in Grand Junction, CO

A Travel Social Worker Licensed Clinical Social Worker will support patients and families in a home-based hospice setting on a 6-week contract in Grand Junction, Colorado. The role serves patients in home, nursing home, and assisted living settings across a rural community with an interdisciplinary care team.

• Provide psychosocial support to patients and families
• Educate families on community resources and care options
• Coordinate with the interdisciplinary care team on care plans
• Advocate for patient goals and wishes as needs change
• Support patients in home, nursing home, and assisted living settings
• Document care activities and follow through on referrals

• Current active Colorado Licensed Clinical Social Worker license
• Minimum 2 years of healthcare experience
• Strong interpersonal and communication skills

• Experience working with older adults, paid or volunteer
