Travel Social Worker in Everett, WA

A Travel Social Worker Licensed Clinical Social Worker will support case management and discharge planning in an acute care hospital on a 13-week contract in Everett, Washington. The role uses Epic and supports an average case load of 18 to 24 patients in a level II trauma center.

• Assess psychosocial needs and barriers to discharge
• Coordinate care plans with nurses, providers and other team members
• Complete discharge planning and care coordination for inpatient cases
• Arrange referrals to home care, DME, rehab and long term care resources
• Document evaluations, plans and patient progress in the medical record
• Support guardianship, advance directive and protective services needs

• Washington state Licensed Clinical Social Worker license in hand
• Master of Social Work degree
• 1 year of acute care experience
• Epic charting experience
• Previous travel experience
• BLS certification

• AIDA EMR experience
• Emergency department case management experience
• ICU, L&D, Mother-Baby or NICU exposure
• Cultural competency and communication support for diverse patients
