Travel Social Worker in Everett, WA

A Travel Social Worker Licensed Clinical Social Worker will support discharge planning and care coordination in an acute care hospital for a 13-week contract in Everett, Washington. The role serves adults and geriatrics in case management, uses Epic, and supports a Level II trauma center. First-time travelers are welcome.

• Conduct biopsychosocial assessments
• Coordinate discharge planning and care transitions
• Collaborate with interdisciplinary team members
• Arrange home care, DME, and transportation resources
• Support guardianship, Medicaid, and Medicare needs
• Refer patients to rehab, long term care, and community resources
• Document assessments, plans, and patient progress

• Washington Licensed Clinical Social Worker or Licensed Master Social Worker license
• Master of Social Work degree from an accredited program
• BLS certification
• 1 year of acute care experience
• Discharge planning and care coordination experience
• Epic charting experience

• Advance directives familiarity
• Adult protective services and high-risk intervention experience
• Emergency Department, ICU, L&D, Mother-Baby, or NICU experience
• Cultural competence and communication support for hearing or speech needs
