Travel Social Worker in Charlottesville, VA

A Travel Medical Social Worker with a Master of Social Work will provide case management and discharge planning for adults and geriatrics in an acute care hospital in Charlottesville, Virginia on a 13-week contract. Epic is used for charting in a short-term acute care setting.

• Assess psychosocial needs and discharge barriers
• Coordinate care plans with the interdisciplinary team
• Support discharge planning and post-acute placement
• Arrange community resources and follow-up services
• Document assessments and interventions in Epic
• Provide crisis support and patient education

• Master of Social Work
• 3 years of acute care hospital social work experience
• Epic charting system experience
• Care coordination and case management skills
• Discharge planning skills

• Medical surgical experience
• Needs assessment and durable medical equipment ordering
• Plan of care development
• Teaching hospital experience
• Trauma Level I or Trauma Level II experience
