Travel Social Worker in California

A Travel Social Worker Master of Social Work will provide social work and discharge planning support for adults and geriatrics in an acute care hospital in Burbank, California on a 13-week contract. Caseloads can reach 25 patients, and charting is in Epic.

• Assess psychosocial needs and discharge barriers
• Coordinate discharge planning with the care team
• Support patients with substance use, homelessness, and mental health concerns
• Organize family meetings and participate in care conferences
• Assist with advance directives, POLST, and protective services needs
• Develop care plans and arrange community resources
• Document social work interventions in Epic

• California Licensed Clinical Social Worker license or Master of Social Work degree
• 2 years of acute medical social work experience in social work and discharge planning
• Epic charting system experience
• Prior travel experience
• Care coordination, collaborative care, and discharge planning skills
