Travel Social Worker in Albany, OR

A Travel Hospice Medical Social Worker will support patients and families in a home setting on a 13-week contract in Albany, Oregon. Care may extend to private homes, skilled nursing facilities, assisted living facilities, and adult foster homes, with a focus on counseling, symptom support, and bereavement care.

• Assess psychosocial needs of patients and families
• Provide counseling and emotional support during end-of-life care
• Coordinate hospice social services and community resources
• Support care planning and transitions across home and facility settings
• Document assessments, interventions, and care updates
• Assist with grief support and bereavement follow-up

• Oregon LMSW or CSWA licensure
• Healthcare Provider Level BLS
• Valid driver's license and auto liability/property damage insurance
• Personal vehicle in good working order
• 1 year of social worker experience

• Oregon LCSW licensure
• Previous hospice experience
• Hospital or medical social work experience
