Travel Social Worker in Albany, OR

A Travel Hospice Medical Social Worker will support patients and families in the home setting during the final stages of illness on a 13-week contract in Albany, Oregon. Care includes medical social services, counseling, bereavement support, and coordination of symptom management with medications and equipment as needed.

• Assess psychosocial needs and family support resources
• Provide counseling and grief support
• Coordinate care with the hospice team
• Help patients and caregivers with community resources
• Document social work visits and care plans
• Support advance care planning and end-of-life needs

• Oregon LMSW or CSWA licensure
• Healthcare Provider Level BLS certification
• 1 year of social work experience
• Valid driver's license and auto liability/property damage insurance

• Hospital or medical setting social work experience
• Oregon LCSW
