A Travel Physical Therapist will evaluate and treat patients in a home health setting on a 13-week contract in Sunnyvale, California.

• Evaluate patients and develop treatment plans
• Provide skilled physical therapy interventions in the home
• Perform admission and initial evaluation visits
• Document findings, goals, and discharge plans
• Coordinate OASIS scoring within established timelines
• Supervise Physical Therapy Assistants as needed

• Active Physical Therapist license in California
• BLS/BCLS certification
• 1 year of home health experience
