Travel Physical Therapist in San Ramon, CA

A Travel Physical Therapist will provide home health visits on a 13-week contract in San Ramon, California. The caseload includes county-based visits in nearby communities and OASIS recertification, resumption of care, evaluation, and discharge visits.

• Evaluate patients in their homes
• Develop and update therapy plans
• Provide treatment to improve mobility and function
• Document visits, progress, and care plans
• Teach patients and caregivers home exercise and safety
• Coordinate care with the interdisciplinary team

• Current active California Physical Therapist license
• BLS/BCLS, ACLS, PALS, and NRP
• 1 year of physical therapy experience
• Graduation from an accredited Physical Therapist program
