Travel Physical Therapist in Petaluma, CA

A Travel Physical Therapist will provide acute care rehabilitation in a hospital setting on a 13-week contract in Petaluma, California. The caseload includes adolescents, adults and geriatrics, with Epic charting and mostly med surg and hospitalist referrals. The role may also include mixed inpatient and outpatient care.

• Evaluate mobility, strength and function
• Develop and update plans of care and goals
• Provide gait training, transfer training and therapeutic exercise
• Recommend assistive devices and discharge needs
• Educate patients and families on home exercise programs
• Document care in Epic and coordinate with the care team

• California Physical Therapist license
• Basic Life Support certification
• 2 years of acute care physical therapy experience
• Epic charting system experience
• Graduation from an accredited Physical Therapy program
• Strong time management and independent practice skills

• Community hospital experience
• Cardiopulmonary rehabilitation experience
• Post stroke rehabilitation experience
• Adult inpatient and outpatient experience
