A Travel Physical Therapist will provide home health care in San Ramon, California on a 13-week contract. The role manages visit-based therapy, treatment planning, and documentation for patients in the community.

• Evaluate patients and develop treatment plans
• Provide therapeutic exercises and mobility training
• Reassess progress and adjust plan of care
• Document visits, outcomes, and patient education
• Support start-of-care, recertification, and discharge visits
• Coordinate care with physicians and other clinicians

• Current active California Physical Therapist license
• BLS, ACLS, PALS, and NRP certifications
• 1 year of Physical Therapist experience
• Valid driver's license, auto insurance, and auto registration
