Travel Physical Therapist in San Ramon, CA

A Travel Physical Therapist will provide home health care to patients in San Ramon, California on a 13-week contract. The role supports territory-based visits, documentation, and point-based productivity in a community setting.

• Perform patient evaluations, reassessments, and discharge visits
• Develop and update treatment plans for home health patients
• Deliver therapeutic exercise, gait training, and mobility education
• Document visit findings and communicate with physicians and care teams
• Manage a territory-based caseload and complete assigned visits
• Provide care for patients with COVID-positive or PUI status when assigned

• Active California Physical Therapist license
• BLS, ACLS, PALS, and NRP certifications
• 1 year of Physical Therapist experience
• Valid driver's license
• Current auto insurance and vehicle registration
