Travel Physical Therapist in Santa Rosa, CA

A Travel Physical Therapist will assess and treat residents in a skilled nursing facility on a 13-week contract in Santa Rosa, California. The role supports rehabilitation, mobility, functional independence, and coordinated care with an interdisciplinary team.

• Evaluate residents' physical abilities and mobility
• Develop individualized treatment plans
• Deliver therapeutic interventions to improve strength and function
• Educate residents and families on exercises and safe movement
• Coordinate care with nurses, physicians, and therapists
• Document assessments, progress, and treatment plans accurately
• Monitor outcomes and adjust plans as needed

• Active California Physical Therapist license
• Degree in Physical Therapy from an accredited program
• 1 year of physical therapy experience
• Knowledge of therapeutic modalities and adaptive equipment
• Strong communication, interpersonal, and problem-solving skills

• Skilled nursing facility experience
